is executed on the date indicated below in
favor of Paul Howard Productions and / or SunnySantaCruz.com, it's agents, affiliates, principals, assigns,
successors, licensees, and
any business entity with which Paul Howard Productions and / or SunnySantaCruz.com is affiliated or beneficially
owns or may
own any interest in whatsoever (collectively referred to as Paul Howard Productions and / or SunnySantaCruz.com ). I irrevocably
and forever authorize Paul Howard Productions and / or SunnySantaCruz.com to publish in print or on the Internet, air on television, broadcast
publicly and on the radio or stream over the Internet, upload to YouTube, reproduce and distribute or sell on video, CD, DVD, MP3 or Download, copy,
manipulate, alter, display, exhibit, sell, license, lease, or convey in any way and at any time and any number of times and anywhere throughout the world
any or all images of me (or my property) or audio from me or about me (or my property). I am 18 years of age (or older) at the time I am signing this
release or have a guardian signing on my behalf who is 18 years of age ( or older ). If submitting video that you own and authorize
Paul Howard Productions and / or SunnySantaCruz.com to use, please clearly print your contact information, complete and sign this release, and mail it along
with your video content to: Paul Howard Productions, Box 4000, Felton, CA 95018. All tapes / media / content received become the property of
Paul Howard Productions and / or SunnySantaCruz.com. No tapes / no media / no content will be returned. Video content used in any video, promo, TV show episode
or any production produced by Paul Howard Productions and / or SunnySantaCruz.com may receive recognition in the show credits. No money shall be paid now or in the future for video used in
any episode produced by Paul Howard Productions and / or SunnySantaCruz.com. I am participating
in a Reality TV show by my own choice / will and am in no
way the responsibility of Paul Howard Productions and / or SunnySantaCruz.com and I hold harmless Paul Howard Productions and / or SunnySantaCruz.com
if I become injured on or at a location where Paul Howard Productions and / or SunnySantaCruz.com is producing
a video or TV show.
